Returning Student-Athletes
- An updated insurance card photocopy (front and back) is to be provided to the Sports Medicine Department upon submission of clearance forms.
- Albertus Sports Medicine requires a yearly physical be completed for all returning student athletes. Physical Form
- Returning Athlete Clearance Paperwork
- Update your Online Profile , including questionnaire and updated insurance. (Any issues logging in, please contact your athletic trainer via email)
- Concussion Course | THIS NEEDS TO BE COMPLETED YEARLY | Your completion certificate will need to be uploaded through Sportsware under "forms".
- Complete your Concussion Statement
- Read the Student-Athlete Fact Sheet
- Complete your ADHD Form (if necessary)
Forms can be dropped off in the Sports Medicine mailbox in the athletic center main office or mailed to:
