Returning Student-Athletes 

1. An updated insurance card photocopy (front and back) needs to be provided to the Sports Medicine Department upon submission of clearance forms.
2. Albertus Sports Medicine requires a yearly physical to be completed for all returning student-athletes. Physical Form
3. Returning Athlete Clearance Paperwork
4. Update your Online Profile (update your insurance).
5. Any issues logging in, please contact your athletic trainer via email.
6. Concussion Course | THIS NEEDS TO BE COMPLETED YEARLY | Your completion certificate will need to be uploaded through Sportsware under "forms". 
   
   1. Complete your Concussion Statement
   2. Read the Student-Athlete Fact Sheet
7. Complete your ADHD Form (if necessary)

Forms can be dropped off in the Sports Medicine mailbox in the athletic center main office or mailed to:
