To be completed by Doctor:
- Physical Form
- Complete your ADHD Form (if necessary)
To be completed by Student-Athlete:
- Returning Athlete Clearance Paperwork
- An updated insurance card photocopy (front and back) needs to be provided to the Sports Medicine Department upon submission of clearance forms.
- Read the Student-Athlete Fact Sheet
- Watch both of these videos VIDEO 1 | VIDEO 2
- Complete your Concussion Statement
- Update your Online Profile (update your insurance).
